Terms and Conditions.

This contract establishes the terms and conditions for the “All on 4” procedure at Top Smile performed by Dr. Omar Lugo, which consists of the placement of dental implants and dental prostheses to restore the patient’s teeth.

A.- The patient must undergo an initial evaluation by a Top Smile Clinic professional to determine their suitability for the “All on 4” procedure. In this evaluation, the patient must disclose any pre-existing medical conditions that may affect completion of treatment. If the patient takes medications, they must provide a detailed list of them, including names and doses. This information is essential to consider and take the necessary preoperative measures to ensure the safety and effectiveness of the treatment. Top Smile is committed to maintaining the confidentiality of this medical information, using it exclusively to provide appropriate and safe treatment to the patient.

B.- Failure to fully and accurately disclose pre-existing medical conditions and medications could put the patient’s health at risk and affect the effectiveness of the “All on 4” procedure. Therefore, the importance of providing truthful and complete information in this initial evaluation is emphasized.

C.- The patient understands and accepts the process of the “All on 4” procedure that includes implant surgery and its subsequent dental restoration. You will be provided with detailed information about the procedure, instructions to follow and care.

D.- For this treatment you will have to make 2 trips. On the first trip we will place a temporary prosthesis, then you will have to wait a minimum of 4 months to place the definitive Zirconia superstructures. This may vary depending on your case.

E.- Top Smile guarantees the quality of the work carried out and offers a document that includes all the conditions and terms.

F.- All the patient’s medical and personal information will be treated confidentially and in accordance with Top Smile’s privacy policies.

G.- This contract will have a complete validity of 12 months from the date on which the first payment is made by the patient. During this period all conditions, terms and prices established in the contract will be valid and applicable. The total price agreed for the “All on 4” procedure will not have any variation or change during the term of this contract, unless another treatment is required outside of the treatment plan initially detailed and agreed upon by both parties. Top Smile is committed to maintaining the agreed price without surprise increases or hidden charges. Any change in price due to the need for additional treatment will be discussed transparently and patient consent will be required before proceeding. This clause ensures that the patient will not face unexpected additional costs related to the “All on 4” procedure while this contract is in force, unless justified by the medical need for additional treatment.

H.- Antibiotics or painkillers are not included in the budget either (Dr. Lugo will prescribe them to you once you are here.

i.- As part of the “All on 4” procedure package, for the First Trip, Top Smile offers the patient a stay at the Hampton Inn by Hilton Cumbres hotel, located very close to the clinic. This stay includes a 6-night stay, one additional guest, and daily breakfast provided at and by the hotel. The length of stay will be determined by the needs of the treatment and agreed with the patient. Top Smile will be in charge of coordinating the reservation at the hotel and will provide the patient with the details related to it. Importantly, this Hampton Inn hotel stay is intended to enhance patient comfort during the “All on 4” procedure process and is provided as an added benefit. Any other expenses or additional services required by the patient will be the responsibility of the patient.

j.- In the event that the patient decides for any reason to cancel or request a refund, Top Smile will retain 20% of the total paid to cover the operating, administrative and accounting expenses derived from the transaction itself.
